Name changes must be submitted in writing, and should include:
- Full Original Name and Address
- Certification Number or Social Security #
- New Full Legal Name
- Copy of Marriage Certificate or Court Order
- Copy of State Issued Photo ID with either old or new name.
- Verification of your current contact information: home address, email, and phone
