Required Components of the Advanced Diabetes Management Application Packet
This is a computer-based test (CBT)
- 1. A completed and signed application for Advanced Diabetes Management Certification
- 2. Copy of professional license or CDR registration (dietitian)
- 3. Copy of membership card (if applicable)
- 4. Copy of official graduate transcript with date of advanced degree conferral
- 5. Course-by-course evaluation of transcript if degree was conferred outside of the United States or in one of its territories (click here for more information)
- 6. A completed and signed Supervised Clinical Practice Verification Form
- 7. For candidates who completed a master's or post-master's program, the Advanced Practice Diabetes Management Educational Preparation form must be completed in its entirety and signed by the current program director, even if you are not a recent graduate.
All applications must be complete and typed or prepared in ink.
